When running the Umbraco editor training course I regularly get the feedback that people (particularly new to Umbraco) find it fiddly to locate the Save button, in that they have to hover next to the Save and Publish button to find it on a hidden menu - Editors annoyed by this maintain that they regular save throughout the creation and editing of a document, and have accidentally published their changes. I always mention that it's better to save using the keyboard shortcut ctrl-S but it keeps coming up time and time again, so thought I would give the issue some visibility on the issue tracker.
I understand the UX thinking that Save and Publish is a more common activity than Save, however I'm not sure that's true for all people in all circumstances.
Similarly, people tend to think of UnPublish as an Action, but it's not available on the Actions menu.
I think people get confused because the Save and the Unpublish options being available on the popup menu above the Save and Publish button when you click the triangle, isn't similar to any of the other of the UI elements in Umbraco? perhaps having the three dots instead of the triangle would be a more consistent way of saying there are more options here... (and a wider clickable target)

Just going through editor feedback on upgrade to Umbraco 7 from Umbraco 6, and again this is reported, see screen grab
Also this will become more of an issue with Variants with V8 - there will be a new important journey of 'Save and notify someone they need to translate'...
