U4-5375 - MNTP start node overridden by user start node

When a user has a Start Node in Content specified, then the start node for a MNTP (either Xpath or specific node) is ignored. I believe the correct behavior is to show the MNTP start node, as long as it is a child of the Start Node in Content.

Example: Content Section A Sub 1 Sub 2 Section B Sub 3 topic i topic ii Sub 4

If MNTP has a start node of "Sub 3", then users without "Start Node in Content" set will correctly see topic i and topic ii when they use the MNTP. However, if a user has "Start Node in Content" set to "Section B", then he will see "Section B" when he uses the MNTP.
